Use it as a conversation
An A3 is more useful when people inspect the work together. Separate an observation from an explanation, then agree on a reversible test. A form cannot establish a root cause or prove a countermeasure worked.
Keep personal data out
Describe process conditions without adding customer identifiers, private staff information or credentials. This worksheet has no autosave or server submission. A downloaded file is under your control; review it before sharing it.
